Guidebook Overview
The Advancing the Care Experience for patients receiving Palliative care as they Transition from hospital to Home (ACEPATH) guidebook is a co-designed guidebook designed for hospital to home transitions. The ACEPATH guidebook was co-designed with patients, family caregivers, healthcare providers, design and health services researchers.
We are currently piloting the ACEPATH guidebook at two study sites to evaluate its acceptability, appropriateness, feasibility, costs, and fidelity. A refined version of the ACEPATH guidebook will be published on our website, once available.

Healthcare Provider Training
To ensure that additional providers within patients’ circles of care are informed about the ACEPATH intervention and the associated implementation study, we developed virtual training materials and sessions. These introduce ACEPATH and explain how healthcare providers can support participating patients and their caregivers, as well as how they can collaborate with study partners facilitating the intervention.
