Have you been the caregiver of a loved one receiving healthcare at the end of life? 

We would like to interview you! 

Our study aims to collect evidence on which aspects of physician care patterns near the end of life enhance or detract from patients’ continuity of care, which aspects of continuity of care are most important to patients and family caregivers, and how continuity of care can best be incorporated into future end-of-life care models. We will use your responses to help us refine the [title] installation, and re-feature it in the future. 

Am I eligible? 

You are eligible if: 

You are a bereaved caregiver (family member or friend who supported the patient at home) of a patient who received healthcare at the end of life  

You were the patient’s main or secondary contact 

You are above 18 years of age, able to speak English or have someone who can translate for you, and have the capacity to consent    

What will I do as a participant? 

If you agree to participate in the study, we will ask you to:  

  • Complete an online demographic questionnaire using Microsoft Forms, which will take approximately 5 minutes.  

  • Participate in one interview, approximately 60 minutes. Virtual or in person (based on your preference)  

You will receive compensation for your time. If you choose to withdraw from the study, you will still receive this compensation.
